Mercy Medical Group, Dignity Health Foundation, Carmichael, CA (1/2014 – 9/2018):

Provide full scope of Internal Medicine outpatient care for complex patients.

United Physicians Inc., New Kensington, PA (8/2000 – 12/2013):

Full scope outpatient and inpatient Internal Medicine practice managing diverse acute and chronic medical conditions.

Inpatient/Hospital:
Attending physician in evaluation, diagnosis, and management of diverse adult medical problems at local hospitals.

Physician Champion for Patient Center Medical Home (PCMH):
Developed strategies to provide multi-disciplinary care for complex patients with multiple medical problems who are at high risk for re-hospitalization and help them get the outpatient resources they require while securing quick post-hospitalization appointment with their primary care physicians.

Served on the Boards of Directors of local hospital and on a number of Committees such as Pharmacy and Therapeutics and Quality Management as well as serving as a Chairman of the Department of Internal Medicine.

Musculoskeletal disorders such bursitis, tendinitis, osteoarthritis, degenerative joint and disc conditions, procedure such as knee, shoulder, and elbow steroid (cortisone) injections
Gastroenterological disorders such as GERD, peptic ulcer diseases, liver and intestine disorders
Neurologic disorders such as different types of headaches, seizure disorder, different neuropathies
Behavioral health disorders such anxiety, depression, PTSD
General wellness and complete physical, pre-employment and school physical exams
Immunization for adult population
His particular interest is in preventive care and management of diseases to prevent future complications

Jasleen A, NP.

Jasleen is a board-certified family nurse practitioner here at Folsom Lake Primary Care and cares for patients 18 and older. Her scope of practice includes management of acute to chronic conditions, providing wellness exams, and routine care. 

She completed her graduate degree in nursing from Touro University, California. Her prior nursing experience revolved around taking care of the elderly and geriatric patient populations. 

Jasleen is passionate about nursing/medicine and wants to spread awareness on health and well-being by increasing education and providing quality care to her patients. It brings her great joy to serve her patients in the primary care setting and build continuity of care overtime. 

 In her free time, Jasleen enjoys spending time with her family and friends.

Aspen Perovich
Physician Assistant

Telehealth Visit

Aspen is a board certified Physician Assistant as well as a Registered Dietitian Nutritionist. She completed her undergraduate and Master’s degree in Human Nutrition at the University of New Mexico and practiced as an RDN for 14 years before going back to school to earn her second graduate degree and PA-C from UC Davis.
She is passionate about primary care and promoting a whole-health approach to medicine. She enjoys caring for patients of all ages and from all walks of life. She has a special interest in preventive care and helping patients learn how to optimize their quality of life through healthy habits.
Aspen lives in Gold River with her husband, three children and their big, furry mutt. In her free time she enjoys cooking, gardening, cheering on her kids at baseball and dance, and bingeing Netflix. She is excited to join the amazing team at Folsom Lake Primary Care.
