Experienced medical providers are available to address your health care needs.
New patients: Please arrive 30 minutes prior to your scheduled appointment time. Remember to bring your insurance card, photo ID, and a printed, completed copy of the new patient packet (linked below). Completing these steps in advance will streamline your check-in process.
Returning patients: Please arrive 15 minutes before your appointment and bring your insurance card to each visit.
All patients, including those using EZ Arrival Check-in, are required to check in at the front desk.

A Message from Dr. Firoz
It is an honor to have your trust to care for your medical needs. I am a resident of Folsom and feel a great sense of community here. That is one of the primary reasons why I chose to start practicing in this beautiful area. As a husband and a father of four beautiful children, I understand the importance of family and community needs, and the value of a one-on-one personal relationship with my patients.
I have worked in both smaller offices and clinics, as well as large multi-specialty groups operating with multiple colleagues in my field and sub-specialists within the same organization. Likewise, I have worked in smaller community hospitals in addition to large university setting hospitals.
While there are a few advantages to working in large groups and hospitals, I feel that it is often challenging to develop a long-lasting and robust rapport with patients, an aspect of medicine that is very important to me. I have seen how this physician-patient relationship can lead to a greater sense of trust, and ultimately a higher level of confidence in care and adherence to treatment. For this reason, I have decided to start practicing medicine “the old way”, where patients and physicians got to know each other on both a professional and personal level.
I am looking forward to meeting each and every one of my patients, and I consider it a privilege and honor to be able to care for your health care needs.

Walk-In’s
To ensure that your needs are addressed we would like to encourage you to utilize the SDV slot as opposed to walking in. There is no guarantee that walkins be seen the same day
After Hour Service Available
While 24/7 phone service is available, please note that after hours phone call and messages are for urgent and emergency reasons only. Please avoid using this service for routine requests. We will be happy to address your routine and non-urgent requests during regular office hours.
Same Day Appointments
We will generally have SDV slot on daily schedule. Please call at opening of the office to reserve that slot.
New Patient
We are accepting new patients and to better serve your needs please call our office or fill out the contact form to make an appointment today.
Prescription Refills
Please call your pharmacy for your routine medication refills and allow at least 2 business days for routine refills.
Please have your medication bottle or previous prescription with you when calling our office so you will be able to give all requested information about the medication to our staff.
If there will be a need for change in your prescription you may be asked to make appointment with your doctor to discuss the change.
